The Department for Work and Pensions has broken its silence following a tragic case where a vulnerable mother died by suicide after her benefits were incorrectly cut. Tamara Jade Logan, a make-up artist from Glossop, Derbyshire, was found dead on May 18 last year, with a coroner ruling that a DWP decision significantly contributed to her declining mental health.
Coroner's Findings on the Tragic Case
Senior south Manchester coroner Alison Mutch published a prevention of future deaths report detailing how the incorrect decision to remove Tamara's enhanced Personal Independence Payment (PIP) allowance, combined with inappropriate communication methods, led to devastating consequences. The inquest heard that upon receiving the DWP letter, Tamara's mental health deteriorated further, culminating in the tragic events of May 2025.
DWP's Official Response and Safeguarding Measures
A DWP spokesperson stated: "Our sincere condolences go to Ms Logan's family and friends. Protecting the millions of people we support every year is a priority. We take the Coroner's comments extremely seriously and will provide a full and detailed response."
The department has since implemented several safeguarding measures designed to prevent similar tragedies:
- Introduction of Vulnerable Customer Champions
- Establishment of Customer Experience and Advanced Support Teams (CEAST)
- Creation of Advanced Customer Support Senior Leaders (ACSSLs)
- Updated guidance on home visits for vulnerable claimants
- New stringent measures for claimants with complex needs
The Coroner's Detailed Assessment
Coroner Alison Mutch's report provided specific findings about the case:
- The decision to remove the enhanced payment has been accepted as an incorrect determination
- The communication method was inappropriate given Tamara's known vulnerabilities
- Upon receiving the DWP letter, Tamara's mental health deteriorated significantly
- On the balance of probabilities, the incorrect decision and communication method significantly contributed to her declining mental health and subsequent death
The Manchester Evening News reported that the inquest found the impact of the DWP decision on Tamara was "very significant", highlighting the serious consequences of administrative errors in the benefits system.
