Family's Heartfelt Tribute After Birmingham Hospital Tragedy
The devastated family of a pregnant mother who died in a Birmingham hospital just days after her baby was stillborn have paid a moving tribute, describing her as a loving parent who cherished creating happy memories with her children. Tammy Mackenzie, a 41-year-old mum-of-four, tragically passed away at Heartlands Hospital on August 6, 2023, following the stillbirth of her daughter Khloe.
Inquest Reveals Critical Diagnosis Delays
An inquest into Tammy's death heard that both she and her baby daughter would have been more likely to survive if doctors at the Birmingham hospital had carried out an MRI scan sooner. Assistant coroner Ian Dreelan concluded that opportunities were missed to diagnose Tammy's underlying condition, which could have potentially saved her life.
The court heard that an MRI scan was recommended on July 26 but did not take place until four days later. When the scan was finally performed, it confirmed a diagnosis of small bowel obstruction. Tammy's family believe that delays in both the diagnosis and subsequent surgery played a significant part in the double tragedy.
Rapid Deterioration and Family's Anguish
Following the delayed diagnosis, Tammy's condition deteriorated rapidly. Her daughter Khloe was stillborn on August 1 after an emergency caesarean section was performed in an attempt to save Tammy's life. Despite being transferred to intensive care, Tammy never recovered and died just days later.
Her mother, Jacqueline Mackenzie, expressed the family's profound grief: "Not only did we lose Tammy, but we also lost a beautiful baby, Khloe. We have all suffered in the weeks and years that have led to the conclusion into Tammy's death."
Family's Hopes for Systemic Change
The family have emphasised their hope that lessons will be learned from this tragedy to prevent similar occurrences affecting other young pregnant women. Jacqueline Mackenzie added: "We hope that errors like this never happen to any other young pregnant women again. It is clear from the inquest there were missed opportunities in her care and that leaves us devastated as a family."
Tammy leaves behind four children - Corey, Louie, Savannah and Charlie - who remember their mother as a much-loved friend who enjoyed life and loved to travel. In a poignant development, the family have recently welcomed the birth of what would have been Tammy's first grandchild, a moment her mother should have been present to witness.
Hospital Trust's Response and Safety Changes
University Hospitals Birmingham NHS Foundation Trust, which manages Heartlands Hospital, has acknowledged the inquest findings and expressed their deepest condolences to Tammy's family. A spokesperson stated that a number of changes have already been implemented following an internal review to reduce the risk of similar circumstances occurring again.
The Trust spokesperson told BirminghamLive: "The loss of both a mother and her baby is a profound tragedy, and our thoughts remain with everyone affected. We have taken forward the learning to strengthen our processes and improve the safety and quality of care."
The family have indicated they will pursue a clinical negligence claim against the Trust as they seek justice and ensure that meaningful changes are made to hospital procedures. They remember Tammy for her smile and the happiness she brought to their lives, vowing to never stop thinking about her.
