An investigation has been launched following the death of a convicted sex offender at a Midlands prison.
Andrew Barraclough, aged 61, died at HMP Stafford on May 30, 2024, while serving a 16-year sentence for sexual offences.
Cause of Death and Investigation Findings
A report by the Prisons and Probation Ombudsman (PPO) concluded that Barraclough died from natural causes, specifically a malignant tumour of the lung.
The condition was caused by idiopathic pulmonary fibrosis and chronic obstructive lung disease.
An inquest held in November 2024 confirmed the death was from natural causes.
Review of Prison Healthcare
A separate review into Barraclough's care was commissioned by NHS England for the sex-offenders only prison.
The investigation found that he had received an 'excellent standard' of clinical care during his time at HMP Stafford.
The PPO report stated that this care was 'equivalent to that which he could have expected to receive in the community'.
The clinical reviewer commended healthcare staff for the 'high quality of patient centred care' provided to Barraclough throughout his imprisonment.
The PPO, which carries out independent investigations into all prisoner deaths, has offered its condolences to Barraclough's family and friends.
